LiteWell operates as a direct-pay medical provider. You agree that payment is due at the time of service and that LiteWell does not bill insurance.
If applicable, recurring billing arrangements will be disclosed separately.
LiteWell providers have opted out of Medicare. You agree not to submit claims to Medicare and acknowledge full financial responsibility for services rendered.
The Site is provided “as is” and “as available.” LiteWell makes no warranties regarding accuracy, reliability, or availability of the Site or services.
To the fullest extent permitted by law, LiteWell shall not be liable for any damages arising from use of the Site, including indirect or consequential damages, loss of data, or medical outcomes.
You agree to indemnify and hold harmless LiteWell from any claims, damages, or liabilities arising from your use of the Site or violation of these Terms.
Any dispute arising out of or relating to these Terms shall be resolved through binding arbitration under the Federal Arbitration Act.
